Development of an Intermediate Care strategy in Cheshire

Nature of Work -

Supporting the partnership in Cheshire in establishing an Intermediate Tier Commissioner Group with the purpose of aligning pathways and strategic commissioning objectives.  The project also entails the whole systems modelling of future capacity needs in line with an emerging high level service model being developed by the partners. The project was shadowed by the Brunel University RIGHT project team who are researching into different modelling approaches as they are applied in healthcare.
