We are working with a local health and social care partnership covering a population of over 600,000 to model, align and ultimately integrate strategic planning and delivery for frail older people, including those with dementia.  This innovative work is using the latest data-matching research to add value to the identification of population needs according to a mix of frailty and dementia.
