The  Whole Systems Partnership has supported the Scarborough , Whitby and Ryedale PCT and North Yorkshire County Council through its programme board for older people .  The board has developed a joint approach  to the modernisation of older people services by specifically redesigning systems and processes in the three critical areas of: – internal acute sector pathways of care – developing an intermediate tier of service which offered alternative step up and stepped down functions – case finding an active case management in order to ensure appropriate responses and organisation of service arrangements. The consultancy has facilitated a number of developmental workshops, undertaken critical friend appraisal and an impact analysis through a modelling approach.
